Shipping Information
In order to process your samples in a timely fashion, please follow the steps indicated below:
Sample Information
Label each specimen with patient name, date of birth, date collected and at least one additional
unique patient identifier (helpful to further identify each specimen and track patient history).
Specimen requirements by clinical test.
Patient & Billing Information
Please provide all patient demographic information including Social Security Number (SSN) to assist in tracking patient samples and history.
Please include a copy of all relevant billing information, or mark requisition form for "CLIENT" billing.
Packaging Information
- Please make sure the cap of the specimen tube/vial is secured tightly; additionally sealing
it with parafilm is advisable.
- Place specimen(s) inside absorbent pouch and place pouch inside of biohazard bag.
- Place one (1) refrigerated (not-frozen) cold pack on top of specimen(s) inside styrofoam insulator.
- Replace styrofoam lid.
- Place completed requisition form on top of closed styrofoam insulator and close specimen box.
SHIPPING INFORMATION
For FedEx Service: Place specimen box inside of "FedEx Clinical Pak" and seal. Complete and
attach FedEx Airbill / Billable Stamp.
If shipping on Friday, attach Saturday Delivery Sticker
and mark Airbill for Saturday Delivery. Call CSI Client Service, with tracking number. Call
FedEx (800) 463-3339 to schedule pick-up or CSI Client Service can contact FedEx on your behalf.
